Job Functions
As Admissions and Social Services Coordinator, you are responsible for leading the facilityâs activities in the preadmission, admission, discharge, and business development processes. May include outside contacts. This employee also provides services to respond to the emotional needs of residents and their families. No supervisory function.
Duties and Responsibilities
Conducts tours of facility for prospective residents whether walk-ins or scheduled admissions.
Receives re! ferral and coordinates clinical assessment, screening, and ens! ures appropriate clinical acceptance in a timely manner.

Assist with arrangements and confirmation of all admissions.
Negotiates and obtains prior authorization for rates and levels of care with non-contracted providers.
Obtain rates and level of care from payor on a per case basis when authorized.
Ensure level of care approval obtained and or OBRA screen (PASSAR, MIMR) when indicated.
Verification of payor source, i.e., Medicare, Managed Care, Medicaid, Insurance.
For Medicaid pendings, collect copy of date-stamped application prior to admission. All deviations require prior approval of Executive Director and RVPO.
Complete face sheet, attach to clinical screen, copy and distribute to appropriate departments.
Contact the County Department of Social Service/Medicaid office to verify eligibility and determine estimate of co-pay/family ! participation amount when appropriate.
Tracks acute hospital admission to ensure readmission of patients.
Requirements
Education :
Bachelor's Degree in Social Work required, Master's Degree in Social Work preferred. Two (2) years equivalent experience in social work. Must meet Idaho State/Federal requirements for this position.
Experience
Must posses at least one year experience in Health Care/Business Development/Insurance Familiar with Medical Terminology Demonstrated ability to assess resident needs, develop and implement a plan of care and coordinate the discharge planning process. Knowledge of accreditation standards and compliance requirements. Must possess experience with public and commercial payor sources preferred
Specific Requirements
Must possess the ability to plan and develop new programs.
Must have patience, tact, a cheerful disposition and enthusiasm as well as the willingness to handle diff! icult residents and staff.
Must be able to relate information conce! rning a residentâs condition.
