Job/Position Title : Independent Assessment Provider, ICDE
Primary Purpose:
Complete work as outlined in the Scope of Work in the contract with the Department of Health and Welfare.
Successful candidate will conduct clinical assessments for the Department of Health and Welfare Region children with developmental disabilities who are applying for Medicaid services. The IAP is responsible for initial and annual eligibility determinations by administering and completing DD eligibility process, and establishing a negotiated budget, based on the assessed needs of the client.
Key Responsibilities:
1. Demonstrated ability to work individually and make independent decisions on a daily basis.
2. Skills and ability to complete a variety of math calculations accurately to include addition, subtraction and multiplication. 3. Daily computer use; application of Microsoft Word, Excel! , and Scales of Independent Behavior Revised (SIB-R)
4. Maintenance of comprehensive, detailed participant database on a daily basis. Enter participant information, dates and results of processes completed on a daily basis to ensure data base is kept accurately up to date.
5. Gather and interpret data and participant history to make an eligibility decision based on a clinical assessment.
6. Interface with participants, families, and providers in a professional manner. Provide support during the assessment process by conveying knowledge of process and accurate information.
7. Schedule assessments with participant and families, ensuring that all timeframe requirements as outlined by the contract are met.
8. Ability to effectively manage time and multi-task. Organize schedule and tasks to ensure that all the required steps and documentation of the assessment process are completed accurately, and within the contracted time frames.
9. Maintain participant! files in a global database, ensuring all required documentation is organized and maintained in a secure location.
10. Return all phone calls and e-mails within 48 hour or 2 business days upon receipt.
11. Administer the SIB-R utilizing interview guidelines as outlined in assessors manual. Accurately record respondent answers in the SIB-R booklet. Upon completion of interview accurately add up and score each section following scoring methodology. Enter results into the SIB-R program.
12. Arrive on time and prepared to all assessments and meetings. Adjust caseload and schedule to complete intakes within 30 days. Available to absorb additional cases in the event that an IAP is out ill, on vacation, or otherwise on leave.
Minimum Qualifications:
Qualified Mental Retardation Professional (QMRP) -- meets the qualifications specified in the Code of Federal Regulations, Title 42, section 483.430.
One year experience working directly with pe! rsons with mental retardation or other developmental disabilities.

Licensed as a doctor of medicine or osteopathy, or as a registered nurse; or has Bachelor's degree in one of the following professions: psychology, social work, occupational therapy, speech pathology, audiology, professional recreation, a related human services profession or another health related field and has experience in completing and interpreting assessments.
Preferred Qualifications: Master's degree in a health related field or Licensed Social Worker preferred.
