RN Pre-Access Case Manager

Location: Portneuf Idaho

Description: Portneuf Medical Center is in need of RN Pre-Access Case Manager right now, this position will be placed in Idaho. Screens scheduled, direct and urgent admissions for appropriate level of care and intervenes as necessary when a criterion is not met. In addition, reviews for appropriate Utilization Review d! ocumentation for those who are screened. Assures clinical appropriateness of services and efficient use of resources by monitoring clinical services and financial outcomes of an assigned caseload. Collaborates closely with members of the health care team: physicians, social services and hospital staff to evaluate cases including: criteria/medical necessity and appropriate level of care. To achieve quality clinical and cost effective outcomes, the nurse care manager coordinates specific treatment, cost, and transition targets, identifies barriers to effective treatment and plays leading role in the interface of treatment and resource allocation. Responsible for discharge planning, continuum placement, access control and advocate for the patient while protecting the best interest of the organization. Serves as a leader/resource to staff on best practices related to assigned patient care groups including treatment, clinical documentation, discharge planning, social services is! sues, community resources, referrals and financial information! related to outcomes. Assists staff with clinical documentation program and partners with Health Information Management on assigning working DRG.

Requirements

Must possess a RN degree, as well as current license in the State of Idaho. BSN preferred. To function effectively in this role, must also have at least five years of hospital experience. Case Management, Utilization Review, or nursing management or clinical lead experience is required. Case Manager Certification highly preferred.
