RN Case Manager

Case Managers provide clinically-based case management to support the delivery of effective and efficient patient care. The case manager has the overall accountability for the utilization management and disc! harge planning for patients within the assigned caseload. Collaborates with other members of the health care team to identify appropriate utilization of resources and to ensure reimbursement. Utilizes criteria to confirm medical necessity for admission and continued stay. With the patient, family and health care team, creates a discharge plan appropriate to the patient's needs and resources.
Minimum qualifications include:
Graduation from an accredited School of Nursing with current nursing licensure from Idaho.
Minimum of 3-5 years nursing experience, preferably in the field of case managed population preferred.
May consider less RN experience if demonstrated relevant case management experience can be determined.
Experience in utilization management and discharge planning preferred.
