The Boise VAMC VIRS Coordinator is assigned to Chief of Staff Service, under the Associate Chief o! f Staff for Non-Institutional Care and provides oversight, coordination, leadership, quality and performance measurement and analysis, and reporting of transfer and referral trends or system issues through the utilization of the nursing process. E xecutes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care for patient receiving care through the Patient Flow Program. C omplies with Boise VAÂ's Healthcare System Clinical Practice Guidelines and Clinical Reminder Alerts within VIRS scope of assignment. Care will be provided in the least restrictive, most cost-effective manner, through use of the clinical practice guidelines and clinical pathways. Evidence of health promotion and chronic disease indices will be monitored through the clinical practice guidelines and performance measures. D emonstrates leadership in delivering and proving patient care through collaborative strategies with others. There i! s significant collaboration with community and VISN healthcare! personnel in the transition of veterans into and out of the Boise VA Healthcare System. Day-to-day coordination and oversight of patient movement for inpatient and specialty care between VA and Non-VA/NVCC facilities. Triage and case management of complex cases. Resolving problems associated with the transfer/referral process. Ongoing tracking including use of Excel and Access data base of consultation requests to other VAMCs. Represents the VIRS function and its contribution to flow during external review visits such as SOARS, OIG, or Joint Commission. Recommends to providers the direction and redirection of services when backlog increases in specialty areas. Alerts COS to specialty care trends on request and when concerns arise. Acts as the contact person and facilitator working closely with veterans and service lines to ensure smooth transitions in transfers and specialty care movement. Participate in Boise VAMC relevant forums, committees, or councils as need indicates! and in VISN20 forums as requested/required. Processes all Interfacility consults ensuring that necessary films, labs and outside paperwork associated with the referral are communicated to the specialty clinic. Participates in required Boise VAMC meetings to include Morning Report and the NVCC Steering Committee. The incumbent will attend other meeting as required and serve as requested on Working Groups when such needs arise. The incumbent will prepare reports as regularly required or as requested by Tetrad members. In addition the incumbent will respond to all suspenses and taskers received as approved by the Tetrad.

Summary
This position is located in the Chief of Staff Service, under the Associate Chief of Staff for Non-Institutional Care, Boise VA Medical Center, Boise, ID. Visn Integrated Referral Services ( VIRS) Coordinator participates in the coordination of care amongst VA ! and non-VA facilities focusing on patient transition through the contin! uum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. The incumbent considers all characteristics of the individual, including age and life stages, state of health, culture, values, and previous experiences. VIRS coordinator i nfluences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs.

Registration - Current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia . The RN must maintain a current, full active and unrestricted registration to continue employment with the VA
Minimum education and ! experience of: 1) Approximately 3 years of nursing practice; 2) Bachelor of Science in nursing (BSN) degree and approximately 2-3 years of nursing practice; 3) Master of Science in nursing (MSN) degree and approximately 1 year of nursing practice; 4) Masters degree in a related field, MSN, and approximately 1 year of nursing practice.

May occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Must be a mature, flexible, sensible individual capable of working effectively in stressful situations, and able to shift priorities based on Veteran needs.
