Location: Meridian Idaho

Assess and engage members for disease management program(s). This includes planning, coordinating, implementing, monitoring and evaluating the medical services required to ! meet the complex health needs of members enrolled in AmeriBen Disease Management program in order to maximize each member's capacity for self-care, to cost-effectively achieve desired clinical outcomes and to enhance quality of medical care.

DUTIES

Key Result Areas

  • Establish personal contact with patients and family members
  • Assess the patient's current status, prognosis, future care needs, treatment plan and alternative treatment plans
  • Coordinate care with case management Utilization Management Staff.
  • Coordinate interventions with the members physicians.
  • Develop and implement individualized Disease Management plans
  • Evaluate effectiveness of care plan and progress made by patient
  • Document findings and appropriate cost effective recommendations
  • Capture and record any cost savings
  • Act as a liaison between referral source, physician and member
  • Maintain confidentiality! of records and information
  • Maintain and report appli! cable statistics regarding programs and patient services
  • Document ongoing education and engagement
  • Attend weekly on-site meetings
  • Assist Marketing as well as Account & Business Development with projects related to promoting and selling our Disease Management services
  • Performs all duties within the scope of her/his licensure.
QUALIFICATIONS

Minimum Qualifications
  • Current unrestricted Idaho R.N. license. Bachelor Degree in nursing preferred.
  • Clinical experience as well as a general understanding of clinical program design, implementation and management/monitoring to support choice in consumer medical care. Must understand the medical utilization implications of such programs
  • Knowledge of HIPAA and DOL regulations
  • Type 45 words per minute and experience working in Microsoft Word, Excel and Power Point.
  • Strong interpersonal relationship skills and the ability to work effecti! vely with a diverse patient population.
  • Ability to communicate effectively, both orally and in writing, medical information to health care professionals, employer groups, Brokers, stop loss personnel, internal customers, members and patient families
  • Ability to work independently under general instruction and with a team
  • Time management, Organizational, Research, Analytical, and Records Maintenance skills
  • Negotiation skills
  • Ability to work efficiently, perform multi-tasking, prioritize tasks, and meet tight deadlines
  • Ability to efficiently and effectively perform the Essential Key Result Areas with or without a reasonable accommodation without posing a direct safety threat to others or self
