Home Health Performance Improvement Coordinator, Registered Nurse - RN

Location: Coeur d'Alene Idaho

Description: LHC Group is looking of Home Health Performance Improvement Coordinator, Registered Nurse - RN right now, this position will be placed in Idaho. The Performance Improvement Coordinator reports to the Performance Improvement Manager and is responsible for facilitating overall agency survey readiness and quality outcomes.

Performs bi-annual survey readiness assessments for each assigned agency, facilitates actions to m! itigate findings, and monitors progress to ensure actions are ! sustained.
Assist the agencies during state and Joint Commission surveys by facilitating continued survey readiness, remaining available during survey, assisting in the development of the required action plan, ensuring follow through with actions plans/Evidence of Standards Compliance (ESC)/Measures of Success (MOS).

Maintains current knowledge of regulations affecting the agency and acts as a resource to staff regarding Conditions of Participation, State Minimal Standards, Joint Commission, OSHA requirements.

Visits agencies on-site no less than twice per month, unless there is an extenuating circumstance that is approved by the manager.

Provides face-to-face exit conferences with Branch Manager or designee after each site visit to communicate findings and discuss collaborative plans of improvement.

Ensures agencies are kept abreast of regulatory changes and company quality initiatives by reviewing agency meetings minutes, surveying ! staff, verifying participation on designated calls, and other activities.

Performs internal audit chart audits and facilitates the development of realistic and effective action plans to improve identified opportunities of improvement.

Reviews and monitors Home Health Compare outcome and process measures and is responsible for facilitating improvements to meet or exceed company and /or national benchmarks.

Requirements
Experience Requirements
5 years healthcare experience, preferably home care and /or healthcare quality related experience

License Requirements

Current Driver's License and vehicle insurance, and access to a dependable vehicle

RN License preferred

Skill Requirements

Understanding of CMS Conditions of Participation, Joint Commission Standards and state licensing standards

Effective written and verbal communication skills
Good interpersonal skills
Fiscal responsibilit! y and accountability
Proficient computer skills with knowledge of M! icrosoft Word and Excel
Goal-oriented and ability to affect change
