RN Case Manager

Location: Portneuf Idaho

Description: Portneuf Medical Center is hiring RN Case Manager right now, this position will be placed in Idaho. Detailed specification about this position opportunity kindly read the description below. To assure clinical appropriateness of services and efficient use of resources by monitoring clinical services and financial outcomes of an assigned caseload. Collaborates closely with members of the health care team: physicians, social services and hospital staff on cases that do not meet established criteria/ medical necessity or level of care. To achieve quality clinical and cost effective outcomes, the nurse care manager coordinates specific treatment, cost, and transition targets, identifies barriers to effective treatment and plays leading role in the interface of treatment and resource allocation. Acts as a ! resource to staff on best practices related to assigned patien! t care groups including treatment and clinical documentation. Assists staff with clinical documentation program and partners with Health Information Management on assigning working DRG.

Requirements

Must possess a RN degree, as well as current license in the State of Idaho. BSN preferred. To function effectively in this role, must also have at least five years of hospital experience. Case Management, Utilization Review, or nursing management or clinical lead experience is required. Case Manager Certification highly preferred.

Classification

Full-time days (80 hr/pp).

Part-time (24 hr/pp)- variable shifts rotating weekends.

PRN â€" variable shifts, no guaranteed hours.
